Dane County local Continuum of Care (CoC) System Performance Measures for FY2024  
as well as the trends in measures over time. Members asked questions and Torrie  
responded.

Sarah Lim from the City of Madison presented an overview of the CoC’s Coordinated  
Entry system and an update on the recent changes to the process and procedures. The  
overview included a history of the system analysis, challenges, and key takeaways the  
planning group discussed.  
The summary of the changes explained the new two-tiered assessment, the two-phase  
implementation of the process in the completed plan, impact & implications, and ongoing  
evaluation.  
Members asked questions and Sarah responded.
